fix(docs): remediate modem trifecta audit findings (#1005) (#1019)

Address all 10 findings from the modem requirements/design/validation
trifecta audit:

F-MD-001 (High): Update design and validation to require ESP-NOW
  status code instead of success flag, matching MD-0506 AC4.

F-MD-002 (Medium): Add MD-0400 traceability to T-0603 and T-0604
  (write and indication relay cover MD-0400 AC3/AC4).

F-MD-003 (Medium): Add MD-0101 traceability to T-0103 (oversized
  `len` triggers RESET-based resync per MD-0101 AC2).

F-MD-004 (Medium): Add MD-0200 traceability to T-0201 (post-ready
  transmit validates MD-0200 AC2).

F-MD-005 (Medium): Add MD-0407 traceability to T-0619
  (`BLE_DISABLE` stops advertising per MD-0407 AC4).

F-MD-006 (Medium): Add MD-0408 traceability to T-0605 (indication
  fragmentation validates MD-0408 AC4).

F-MD-007 (Medium): Expand T-0615 to test both clean disconnect
  (reason `0x16`) and error disconnect (reason `0x13`) per
  MD-0411 AC2.

F-MD-008 (Medium): Fix T-0701 to assert INFO-level failure logging
  per MD-0500 AC2, with additional WARN assertion per design sec 14.3.

F-MD-009 (Medium): Expand T-0800 to assert input-mode configuration
  at boot (MD-0600 AC1) and add T-0800a for LOW-when-pressed
  (MD-0600 AC3).

F-MD-010 (Low): Add explicit MD-0416 reference to the tentative
  accept model note in design sec 15.2.
