test: expand coverage from 76% toward 90% target

Cover previously-missed lines: complete(metadata=...), _process_name attribute
on entity spans, child_span (known/unknown parent, attributes, entity-id
inheritance), Instrument.shutdown(), real Instrument.__init__ paths,
install_context_fields(), _HelixJsonFormatter exc_info branch,
_install_json_handler no-handlers branch, update_log_service_name with
process_name and live log_provider, and _install_otlp_handler body.
