docs(specs): fix mdlint violations in validation log

Restructures the validation log to satisfy the project's mdl ruleset
(blocked CI). Information content is unchanged; only the markup
shape:

- MD040: add language tags to every fenced code block (bash/text).
- MD032: insert blank lines before lists that previously followed
  paragraphs directly.
- MD029: convert numbered lists with multi-line items and emphasis-
  triggering characters to bullet lists (sidesteps the ordered-list
  prefix parser).
- MD033: replace <id> and <path> placeholders that mdl was parsing
  as HTML tags with INSTANCE_ID / PATH wording.
- MD024: rename the duplicate '### Cost actual' header to
  '### Cost summary (2026-05-18 run)'.
- MD012: trim trailing blank lines at EOF.
