chore(seed): add failed-login history so admin sidebar surfaces it

Admin "Failed Logins" link is hidden when the activity_log has zero
AUTH_FAILED_LOGIN rows. Seeds a small mix of attempts on real accounts, a couple of unknown-identifier probes,
and one 4-attempt sequence that ends in a PIN lockout, so the link
is visible in the demo recording.
