fix(caver): fix delete/merge losing data and add missing FK handling

- Remove ghost 'exploredEntrances' association (legacy from j_entrance_caver)
- Validate entityId as numeric, return 400 for invalid values
- Return 400 when merge target caver does not exist instead of
  silently falling through to no-merge path
- Add missing FK reassignment for TCrs, TPoint, HDocument.validator,
  TTokenBlacklist, and subscribedToRegions collection
- Use replaceCollection to always clear junction tables regardless of
  whether getCaver populates them
- Add audit log on permanent delete for traceability
- Remove legacy exploredEntrances from tcaver fixture
- Add TCrs to ESLint globals
