fix(document): clean up entrances and bibliographic metadata on permanent delete

The permanent delete of a document failed with FK violations because:
- j_document_entrance rows were not removed (entrances association missing
  from linkedEntitiesDeleteOrMerge calls)
- t_bibliographic_metadata rows were not removed before the hard delete

Also extract shared bootstrap logic into test/seed-database.js to avoid
duplicating the seeding sequence between bootstrap.test.js and
snapshot-test-db.js.
