feat(config): add ignore_usernames_override per instance

When defined on an instance (even as an empty list), this option

overrides the global ignore_usernames for that instance only.

Missing key inherits the global list. The filter now runs in the

per-instance dispatch loop instead of the global pre-dispatch path.
