fix(cli): correct skill add doc examples to use flag-first ordering

urfave/cli/v2's default parser stops parsing flags after the first
non-flag arg, so 'holodeck skill add using-holodeck --claude' was
rejected with 'must specify at least one of --claude/...'. Rewrites
the in-binary --help examples and README to put agent flags before
the positional skill name, matching actual parser behavior.

Caught during Team Lead review of draft PR #812.
