fix(ci): lint + coverage gate to 100%

Lint fixes:
- dal/key.go, record/data_with_id.go: rename reflect.Ptr -> reflect.Pointer
  (govet inline rule)
- dbschema/reader_test.go, ddl/alter_op_test.go: remove redundant type
  annotations (staticcheck ST1023) and embedded-field selector
  qualifiers (QF1008)

Coverage to 100% (CI threshold):
- dbschema: tests for ListCollections/ListIndexes/ListConstraints/
  ListReferrers helper dispatch + not-implementer branches, plus
  Type.String("unknown"), backendName(nil/nil-adapter), and
  DefaultLiteral/DefaultCurrentTimestamp sealed markers
- ddl: tests for all six AlterOp sealed markers + backendName branches
- mocks/mock_dal/db_test.go: cover MockDB.SupportsConcurrentConnections

New mock package:
- mocks/mock_ddl/: gomock-generated MockSchemaModifier,
  MockTransactionalDDL, MockApplier + 100%-coverage tests
