docs: Restructure our charting documentation and add additional content based on decision guide (#9163)

* docs: restructure charting guides and relocate external documents

Consolidate charting into designing-charting, visit-templates, and chart-data-model.
Move external documents to fhir-datastore; broaden structured-data-capture framing.
Add client redirects for legacy charting URLs; update cross-links and anchors.

* docs: refine charting section clarity, flow, and structure

- Rewrite index.md opening to be clinician-first; collapse Reference into See Also
- Tighten designing-charting.md: add scope, adoption note, explicit default
  (structured S/O/P + narrative A), remove duplicate link section, title case
- Restructure visit-templates.md: promote workflow patterns, remove ~4x SOAP
  mapping duplication, merge diagrams under one section, slim Plan section
- chart-data-model.mdx: add navigation sentence, remove empty stub, deduplicate
  User Interface headings, fix code fence, link value-set URLs, label cautions,
  annotate helper-function blocks, fix Symptoms vs Conditions casing
- external-documents.mdx: drop redundant Introduction H2, strengthen lead,
  add bots/creating-a-pdf to See Also
- structured-data-capture.md: add missing frontmatter (title, tags, keywords)
