* docs(dosespot): document self-service prescriber enrollment bot

Add documentation for `dosespot-self-enroll-prescriber-bot` to the
prescriber enrollment guide. Includes a comparison table of both
enrollment approaches, PractitionerRole setup, auto-advancing flow
sequence diagram, Practitioner resource requirements, and input/output
reference.
