Fix exception reference cycles

Fixes #1295.

Clear local exception references when re-raising through OmegaConf error formatting helpers so traceback frames do not retain exception objects through reference cycles. Also avoid self-referential exception causes when full tracebacks are enabled for already-initialized OmegaConf exceptions.

Add regression coverage for frame-local cleanup and full-traceback cause handling.
