feat(scanner): parallelize server scan and lower scan-freq to 120s

Downstream consumers like nanopub-router rely on the monitor's snapshot
to route requests, so hour-long staleness was unacceptable. To safely
shorten the interval, add a 15s socket timeout to bound per-request
worst-case, run server tests through a fixed thread pool
(scan-threads, default 8), and drop scan-freq from 3600 to 120.
