vm: wrap ErrNextElementOccupied as a throwable Error

OpArrayInitAppend / OpArrayInitKeyed returned the raw
ErrNextElementOccupied which the dispatcher then surfaced as a
fatal. The AST's runArray wraps it as phpobj.Error so user
\`try { … } catch (Error \$e)\` can handle it (per the
array_literal_next_element_error.phpt fixture).

Mirror the AST wrapping.
