vm: cache resolved callable per call site

OP_CALL_USER previously called Global.GetFunction on every invocation,
which is a hashtable lookup. Cache the resolved callable in a
function-level slice indexed by the name's const-pool position.
Lazy-allocated on first hit; never invalidated (function redeclaration
is rare and out of scope for the MVP).
