fix: resolve ReadTheDocs build errors and RST docstring warnings

- Fix code-block directive in getting_started.rst (missing blank line)
- Exclude OOM/file-dependent notebooks from RTD execution
  (physiological_features, advanced_computation, data_loader_tutorial,
  feature_engineering_autonomic)
- Remove deprecated canonical_url; use html_baseurl in conf.py
- Expand suppress_warnings for myst.header and ref.duplicate_label
- Fix RST substitution ERROR in transfer_entropy.py (pipe chars in abs())
- Bulk fix 128 source files: Examples: underline too short (8→9 dashes)
- Fix inline markup warnings: slash in bold, Markdown fences, bare asterisk
  in autoencoder.py, duplicate [1] citation keys in respiratory modules
- Fix ml_models bugs: ConvolutionalAutoencoder super() delegation,
  decoder length mismatch, pretrained model network isolation in tests
- Add vitalDSP_agent module scaffold with architecture docs
