Extend ADR-0002 with non-chatbot client section

  The original ADR scoped prompt-extension delivery to the chatbot UI,
  leaving direct MCP clients (Claude Code, ad-hoc agents) without a
  bootstrap entry point. Adds §5 covering `system_primer`, dual-registered
  as an MCP prompt (slash command) and an MCP tool (auto-discovery via the
  LLM tool list), plus the FastMCP server `instructions` field as a
  reinforcing directive. Strategy placement clarified: selection logic
  stays client-side since model tier and token budget are unknown to the
  server. Phase 4 added for the primer; open question on `applies_to`
  evaluation expanded to cover server-side filtering.
