feat: add PaymentRequest entity, migrations, and stripe link (#895)

Introduces the PaymentRequest primitive (#639): a UUID-keyed entity that
represents a fixed-amount, expiring, shareable request to top up a user's
balance. Status is derived from paidAt/cancelledAt/expiresAt rather than
stored.

Adds two migrations: one creating the payment_request table, one altering
stripe_payment_intent to add the optional paymentRequestId back-reference
that lets the Stripe webhook flip a request to PAID on success.
