fix: resolve CI lint failures and npm bin gitignore gap

- Fix 3 errcheck violations in serve.go (unchecked fmt.Fprint returns)
- Fix staticcheck S1011 in migration.go (replace loop with append)
- Remove unused webFile type in web.go
- Add .gitignore exception for npm/bin/ so npm/bin/rtmx is tracked
- Make pre-commit hook fail (not warn) when golangci-lint is missing

Root cause: golangci-lint was not installed locally, so lint was
silently skipped. npm/bin/rtmx existed locally but was excluded by
the bin/ gitignore rule, causing TestNpmInstall to fail in CI.
