rename rates() to rateAndTransitTimes(), accept full request body verbatim

Breaking changes:
- rates(requestedShipment, options) → rateAndTransitTimes(rateRequest, options).
  The caller now supplies the full request body — accountNumber,
  requestedShipment, and any of rateRequestControlParameters, carrierCodes,
  processingOptions, version — rather than only requestedShipment.
- Drop account_number from constructor and _options.account_number. Pass
  accountNumber in the request body instead.

In-body error handling now concatenates every "code: message" from errors[]
with "; " separators so callers see the full picture, not just errors[0].
