fix: align MCP submission list output and improve coverage

Motivation:
The submission list output in the MCP tool was misaligned, with the project
name appearing under the wrong label. Coverage for channel handling was also
incomplete.

Design Choices:
Fixed the sprintf format string and argument order in the list_submissions tool.
Refactored channel text generation to be more defensive and easier to cover
with tests. Added POD for the server method.

Benefits:
Improved output quality for MCP users and better code maintainability through
improved test coverage.
