test: cover command-processor failure routing

where a command ends up (failed/ vs done/) depends on the
break point — parse, schema, executor, markDone. None of those
branches were tested directly; a regression could silently
re-execute a command or drop one. Also covered: duplicate
watcher event noop, parse-retry success on truncated read.
