fix(contracts): default futures exchange to CME, not GLOBEX (#546)

IBKR renamed GLOBEX to CME several years back; current TWS/IB Gateway
returns error 200 ("No security definition has been found") for any
request specifying GLOBEX as the exchange. `Contract::futures(...)` and
`Contract::continuous_futures(...)` shipped with `"GLOBEX".into()` as the
default exchange in `FuturesBuilder::new` and `ContinuousFuturesBuilder::new`.

Also opportunistically updates the deprecated `ContractBuilder` doc-examples
and the `docs/contract-builder.md` `Exchange::from(...)` cookbook.

Closes #544.
