chore: update github-app-token action

Replaces the deprecated `tibdex/github-app-token` with the official
`actions/create-github-app-token@v3`.

- Updates input parameters to use `client-id` and `private-key`.
- Updates the application ID secret reference to `APP_CLIENT_ID`.
