fix: force vertical MCP tools layout and improve CSV diff contrast (#108)

MCP diagram: add invisible links (~~~) between T1-T7 to force vertical
stacking -- direction TB alone is ignored for unconnected nodes in
flowchart LR.

CSV diff diagram: darken header fill (#d1d5db), context fill (#e5e7eb)
and use near-black text (#111827) on context row. Previous #f3f4f6 fill
with #374151 text was nearly invisible.
