disable autosens and AMA by default in oref0-setup.sh (#212)

* #210: Validate carb ratios are reasonable

* fix comment; high carbRatio is safe: it will just make COB 0

* disable autosens and AMA by default

* delete optional arguments from determine-basal device

* require an enter to Continue

* require an enter to schedule cron

* don't switch oref0 branches

* add enable to Usage

* add NIGHTSCOUT_HOST and API_SECRET to crontab for NS upload

* use the shell $PATH instead of hard-coding one
