ci: bump actions/checkout to v6 and codeql-action to v4

Two Dependabot updates rolled into one CI-only commit:

- actions/checkout v5 -> v6 across all 7 workflows (8 occurrences).
- github/codeql-action/init and /analyze v3 -> v4 in codeql.yml.

Both are major bumps but no breaking changes for our usage:
checkout@v6 is a runtime modernization (Node 24); codeql-action@v4
drops Node 16 support (we never opted in). All actions still work with
the same `with:` keys we already pass.
