feat: disable deblocking by default, add per-MB PSNR tool

The deblocking filter destroys 6-14 dB of PSNR on block-constrained
content because it interprets intentional block boundaries as
compression artifacts. Replace -no-deblock with -use-deblock so
deblocking is off by default in hi264gen.

Add cmd/psnr tool for per-macroblock PSNR comparison of YUV files.
