docs: document tool-swap setup and list missing wrappers

Add a "Swapping Implementation and Review Roles" section under
Configuration covering the Codex-implements / Claude-reviews pattern
and similar role-flips. The section explains how `claude_command`,
`external_review_tool=custom`, and `custom_review_script` combine,
points at scripts/opencode/opencode-review.sh as a working custom
review script template, notes that phase log labels reflect the slot
rather than the underlying tool, and warns about committing per-project
.ralphex/ config on the default branch before creating feature branches
so reviewers do not see infrastructure files as new.

Also expand the existing "Working examples are included" list under
"Using Alternative Providers" to include the gemini-as-claude and
opencode wrappers, which were already shipped but undocumented.

Addresses friction points raised in #330. Other points there
(brew bundling, CODEX_BYPASS env var, --review move-to-completed,
runtime tool-version reporting) declined separately.
