fix: persist DbHandler connect backoff across pool restarts (#964)

The cooldown timestamp lived in TenantCache, which dies with the
TenantSupervisor. After a pool restart it was always nil, so new
DbHandlers reconnected immediately. Move it into a new app-level named
ETS table that outlives the pool.
