fix: return ErrBadConn in more cases (#98)

The driver now returns driver.ErrBadConn in all required cases. With
this, database/sql.DB methods correctly retry getting connections out of
the pool and are more likely to succeed if there are intermittent
connection issues.

The fix allowed workarounds in TestIntegration_Restart to be removed.

The PR also includes minor unrelated documentation and testing
improvements for handling SET statements.
