feat(provider): rename IProvider methods and change ScopeAccessRule to reducer

BREAKING CHANGE: `IProvider.appendArgs`/`appendArgsFn`/`setAccessRule` renamed to
`addArgs`/`addArgsFn`/`addAccessRule`; `ScopeAccessRule` now takes `(prev: boolean,
options: ScopeAccessOptions) => boolean`, enabling multiple access rules to compose
as a left-fold starting from `true`.
