Fix release workflow so "What's Changed" reflects the actual diff

The previous action created its own date-based tag and ignored the
pushed version tag, then auto-generated notes against the last
published (non-draft) release. Because every release was created as
a draft, each new run kept comparing against v0.1.3 and emitting
the same PR list.

Use `gh release create` directly so the release is anchored on the
pushed tag and the notes are generated from the previous tag.
