* fix(ci): run upstream sync weekly and skip when PR exists

The nightly sync was force-pushing sync/upstream-main every day,
clobbering manual conflict resolution work in progress. Two fixes:

- Change schedule from daily to weekly (Monday midnight UTC)
- Skip the entire sync job when an open sync PR already exists

Can still be triggered manually via workflow_dispatch at any time.

* fix(ci): add base filter and concurrency guard to sync workflow

- Add --base filter to PR lookup so it only matches PRs targeting the
  same downstream branch (avoids false positives from other head refs)
- Add job-level concurrency group to serialize runs and prevent
  check-then-act races between concurrent workflow dispatches

SHA-pinning of third-party actions skipped — inconsistent with rest of
repo which uses mutable tags throughout.
