Rename base package to content and create layer entries (#2795)

## What changed?

After some thought, I think it makes more sense to require applying base styles via a core entry since all components rely on those styles to render properly. As part of this refactor, the "base" package bas been renamed to "content" to help distinguish it from core's base styles output but also to be more clear what it's purpose is.

Lastly, this PR creates the necessary entries for both SCSS and CSS in `core` and the `vrembem` package itself. The vrembem package itself now has entries that map cleanly to the layer definitions of "tokens", "base", "components", and "utilities".

**Additional changes**

- `docs`: Documentation has been updated to reflect the above changes.
