refactor: add parent to Requirement and rewrite mergeParameters with Map

- Requirement.get() now sets child.parent = this so consumers can navigate
  to the logical parent without URL string parsing
- getEffectiveParameters() in OperationTypeCoder uses this.requirement.parent
  instead of deriving the parent path from the URL
- Merge logic in both getEffectiveParameters() and mergeParameters() (dispatcher)
  now uses a Map keyed on "${in}:${name}" instead of Array.findIndex
- Added requirement tests for the new parent property
