Fix double escaping in "Monthly Call Limit Reached" message

The alert text was first escaped in call_message.html,
and then escaped the second time in phone-call-limit-body.html.

The fix is:

* Use "|safe" in call_message.html so it produces unescaped content
* The Twiml field needs it escaped (since it is a XML document).
  Escape it using xml.sax.saxutils.escape.
