chore: Add anti-capitulation rules to default persona (#589)

Adds a new `challenge_handling` section that instructs the assistant to
classify user pushback before responding — distinguishing between new
information, surfaced costs, and conviction tests — and to defend its
position when no new evidence is presented.

Adds a `Capitulation Anti-Patterns` behavioral rule that enumerates
specific phrases and openers that mark sycophantic capitulation (e.g.
"You're right", "Fair point", "Good catch") and instructs the assistant
to stop and apply the `challenge_handling` classifier before using them.

Refactors existing sections to cross-reference the new rules: the
`core_principles` section now delegates pushback handling to
`challenge_handling` and uncertainty handling to
`errors_and_uncertainty` rather than restating guidance inline.

Also consolidates the previously separate "Default Style", "When To Be
Detailed", "Writing Voice", and "Formatting Discipline" response style
sections into two denser sections — "Response Length" and "Voice and
Formatting" — reducing redundancy without losing any rules.
