refactor: restructure comms-adapter tests to follow BDD describe/when/it convention

Align the test structure with the repo's established pattern (e.g.
bans-adapter, comms-component, access-checker) using nested describe
blocks with 'when' for context and 'and' for conditions, beforeEach
for shared setup, and it blocks with only assertions.
