Make wide_ops more portable (#162)

This improves several things:
- Use `BEMAN_BIG_INT_HAS_INT128_FUNDAMENTAL`
- Rely less on `wider_t` for portable implementations, especially considering that `wider_t` doesn't always provide the best codegen and is itself not portable.
- Use `BEMAN_BIG_INT_HAS_CONSTEXPR_BUILTIN` to make use of `__builtin_addc` during constant evaluation.
- General cleanup.
