Drop required PR approvals from the default ruleset

The default ruleset was requiring 1 approving review on every PR
targeting the default branch, which on solo-maintainer chrisns repos
just blocks self-merge without adding any actual review value. Set
required_approving_review_count to 0 — PRs are still required by the
ruleset, but the maintainer can self-merge without waiting for an
approval. Drop dismiss_stale_reviews_on_push and require_last_push_approval
since both become moot at 0 reviews.
