Add playoff bracket, series schedule page, and UI polish

- API: playoff bracket and per-series schedule JSON; standings fallback when empty
- Routes: /playoff-series/... page, /api/playoff-bracket, /api/schedule/playoff-series/...
- Home: bracket with links to series pages
- Series page: scores-style game cards, blue header, team links, series lead and
  clinch copy, full team names in header, next-game line sizing
