fix(system-prompt): add link uniqueness rule to rule 7C

  The VIOLATION self-check was incorrectly prompting re-linking of
  identifiers already used as URL reference markers in the same sentence.
  Adds an explicit LINK UNIQUENESS principle alongside the existing
  CORRECT/ANTI-PATTERNS guidance: each URL appears at most once per
  sentence, and link references must not be repeated within it.
