add WARN on InstanceID divergence + regression test for empty-blob branch

Address review:

- log a [WARN] when the persisted blob carries a non-empty InstanceID
  that differs from the CLI/env value. Behaviour is unchanged (blob
  still wins) but the divergence shifts the runtime gid and the
  Argon2 salt derived in app/config/crypt.go, so the next save would
  re-encrypt sensitive fields under a different key. Surfacing it
  beats silent.

- TestLoadConfigFromDB_PreservesCLIInstanceIDOnEmptyBlob locks in the
  new branch: stages a row whose gid matches the CLI value but whose
  JSON blob has instance_id="", then asserts loadConfigFromDB retains
  the CLI value (without the fix it would be "").
