fix(index): skip files that fail to chunk instead of aborting the run (#139)

A single un-parseable file (e.g. a docs code-example snippet without
a `package` declaration) used to abort the entire indexing run. Treat
chunker errors as per-file skips, mirroring the existing permission-denied
and binary-file handling. Persist the file's real content hash so that
re-runs are no-ops until the source actually changes.

Surfaces a "skipped N files (parse errors — see debug log)" line in the
CLI summary and a `files_skipped` field in the structured log.
