Add AAD methodology documentation and refine Markdown table style rules

- Document the dual-path AAD framework (native expression templates and XAD)
  covering expression template hierarchy, tape design, adjoint propagation,
  multi-output support, and parallel Monte Carlo AAD
- Refine Markdown table column width rules to specify content-width-driven
  formatting (width = longest cell + one leading and one trailing space)
- Mirror new doc and updated rules across .claude/ and .codex/ trees
- Fix missing trailing newline in CLAUDE.md
