fix: Address final round of Copilot AI documentation and test feedback

1. Update documentation to reflect correct flag behavior
   - Remove 'mutually exclusive' language for --open-gh-issue and --open-gh-pr
   - Document that flags can be used independently or together
   - Update flag descriptions in table to show they can work together

2. Fix duplicate test case
   - Replace duplicate 'Should fail when --use-gh-models without --open-gh-pr'
   - Add positive test 'Should succeed when --use-gh-models with --open-gh-pr'

3. Improve flag descriptions for default-true flags
   - Change to standard pattern: 'By default, <behavior>. Disable with --flag=false'
   - Removes redundant '(default: true)' text
   - Follows K8s CLI description standard for opt-out flags

4. Move 'Update completed successfully' log after PR/issue creation
   - Prevents misleading success message if PR/issue creation fails
   - Log now only appears after all operations complete

5. Add missing --open-gh-issue flag to docs example
   - Example workflow now shows both flags (matching defaults)
   - Accurately represents default behavior
