Fan out cron per-installation to dodge 360s timeout

The single cron Lambda iterated every repo across every installation
sequentially. One slow installation (404 storms on repos the App can't
reach) consumed the full 360s budget and the cron never reached most
installations — including chrisns, so cns-blog and friends never got a
consent issue or PR.

Split into a dispatcher that enumerates installations and async-invokes
a worker Lambda per installation. Each installation gets its own 360s
and runs in parallel, so a slow tenant can't starve the rest.
