fix(prompts): keep task section headers in English for localized plans

When users asked for plans in non-English languages, the generator would
translate the `### Task N:` / `### Iteration N:` headers too, and the parser
(which matches on the literal English keywords) then rejected the plan as
having no executable task sections.

Add a CRITICAL RULE in make_plan.txt stating that these headers are
structural tokens for the parser and must stay in English regardless of
plan language. Task titles and body content may still be localized.

Also note the constraint in CLAUDE.md next to the existing plan format
documentation.
