LIBAPPO1-86 Update to Rack 3, stabilize flaky tests (#419)

* Run bundle update, yarn upgrade

* Upgrade Rack to 3.2 and align asset pipeline

- bump rack to 3.x and resolve lockfile updates

- upgrade sass-rails to 6.0 and pin sprockets to 4.2.2+ for compatibility

- remove duplicate upload stream write in FileUploadsController#create

* Stabilize Selenium setup in CI

- add explicit Capybara headless Chrome driver for CI runs

- load spec/support config from rails_helper

- set CI Chrome environment variables in CircleCI and GitHub Actions

* Harden feature test reliability

- add DatabaseCleaner setup for js-enabled specs

- switch multi-value feature spec to Warden login and wait for edit form

- add upload feature coverage for temp file cleanup and sanitized filenames

* Fix CI coverage path and shard attribution

Align CircleCI Coveralls LCOV path with SimpleCov output and give each CI shard a unique SimpleCov command name so parallel test splitting reports accurate combined coverage.
